{Tuberculous meningitis is one of the most common type of Central Nervous System Tuberculosis which is an infection of the Meninges. It is most commonly caused by Mycobacterium Tuberculosis. It can be Acute or Chronic and leads to increase pressure within the skull which results in severe nerve and brain tissue damage. In this case, the patient is having Tuberculous Meningitis along with coexisting illness such as Hypertension, Type 2 Diabetes Mellitus, Hypothyroidism, Chronic Kidney disease and Potts Spine. Appropriate treatment was provided to the patient.}

{Background: A drug interaction occurs when the effect of a drug is altered by other drugs; it may affect the drugs pharmacokinetics or pharmacodynamics. In pharmacokinetic type of drug interactions, a drug alters the absorption of another drug or the, distribution, metabolism, or elimination of another drug; in pharmacodynamics type of interactions, the action of a drug is altered by other drugs. This study was conducted to investigate the prevalence of potential drug-drug interaction, to examine the number of drugs used and prevalence of polypharmacy, to determine the frequency, severity and the drug combinations involved in DDIs occurring in ICU. Materials and Methods: This is an observational study carried out at an ICU of a multi-specialty hospital. Patient of age group between 18-80 years of both the gender admitted for more than 24 hours and prescribed at least more than 2 drugs are included in the study. All the relevant and necessary details of the patients including the demographic data, laboratory parameters and drug therapy details were collected from the patient case notes. The patient drug therapy were reviewed every day and assessed for any drug-drug interactions. Results: A total of 151 prescriptions were assessed out of which 82 were male and 69 were female. In the study group the average age was 54.6 and association was found between the DI and increased age. There were around 1,709 drugs prescribed and average drugs per prescription was 11.2. From a total of 276 drug interactions found, 46 drug interactions were of major severity, 184 drug interactions of moderate severity and 46 drug interactions of minor severity. Mechanism of DDI was identified of which 85(30.7%) DDI showed pharmacokinetic mechanism, 135(48.9%) DDI showed pharmacodynamic mechanism and 57(20.6%) DDI showed unknown mechanism. The prevalence of drug-drug interaction was seen more in age group of 41-60 years. Conclusion: Prevalence of potential DDIs is high in intensive care units and is predisposed by factors such as a number of drugs received by patients, comorbid conditions; complex pharmacotherapies administered and geriatrics. Health professionals who provide care to these patients should be aware to identify and prevent possible drug events. Also, the pharmacists involvement might have allowed greater familiarity of physicians regarding clinically relevant drug-drug interactions, optimizing the quality of the prescriptions, especially in critical care units. Therefore, the outcome of clinical pharmacists applications and intervenes may promote well-being, preventing and monitoring adverse events, intervening and contributing to pharmacotherapy effectiveness and prioritizing patient safety.}

{Introduction: Cardiovascular diseases are the leading cause of death in industrialized countries and are expected to become so in emerging countries by 2020. Among this coronary artery disease is the most prevalent cardiovascular disease. ACS may be defined as all clinical syndromes compatible with acute myocardial ischemia resulting from an imbalance between myocardial oxygen demand and supply. ACS comprises of NSTEMI, STEMI, and unstable angina. Methods: This is a prospective observational study conducted in MAXCURE hospital Hyderabad. Cases were collected who were diagnosed with ACS from inpatients of cardiology department in duration of 6 months. Results: A total of 198 cases of ACS were observed. Among them 61 cases of NSTEMI, 67 cases of STEMI, 70 cases of unstable angina. It was more common in men when compared to women and was found to be more prevalent in patients under the age of 55. Most common medical history of patients was angina and hypertension. All patients underwent surgery either CABG and PTCA. The drugs used were aspirin, clopidogrel, beta blockers, statins, nitrates, ACEI, heparin. Conclusion: ACS is one of the most common coronary diseases. The main symptoms includes chest pain and shortness of breath grade II and III. Maximum patients suffer from unstable angina followed by STEMI and then by NSTEMI. The age group most affecting is less than 55. Males are at more risk than females. The common history of patients include angina, hypertension, diabetes and smoking. All the patients arriving at emergency will be prescribed with aspirin, beta blockers, nitrates, statins, clopidogrel and heparin. The use of calcium channel blockers, ACEI, thrombolytics, warfarin, LMWH, is less or almost nil. }

{Introduction: For decades, intraoperative anemia has been treated with red blood cell transfusions since it was believed that oxygen supply would increase by increasing hemoglobin levels. There is evidence that blood transfusion is associated with adverse events and should be avoid as much as possible. For this purpose, it is essential to know the compensatory physiological mechanisms during anemia. Venous oxygen saturation is a clinical tool that integrates the relationship between oxygen intake and consumption, which is easy to obtain once a central venous catheter, is available. Material and methods: A longitudinal, prospective, observational study was conducted which included patients schedule for elective or emergency procedures that, due to their clinical conditions, had a central venous catheter. A sample of venous blood was taken from the central venous catheter and sent to the laboratory for gasometry. The results were correlated with the clinical status and vital signs of the patient. The following variables were evaluated: vital signs, hemoglobin, hematocrit and oxygen saturation before and after transfusion. Results: 34 patients were evaluated with an average age of 52 years. 58.8% was transfused. Despite the transfusion and variations of hemoglobin, the SaO2 in the pulse oximeter remained without changes pre and post transfusion. In gasometry, a difference between hemoglobin and initial hematocrit and pre-transfusion was observed, this due to bleeding that occurred. No differences in SaO2 values were observed for pre-transfusion vs posttransfusion pulse oximetry. Conclusions: We found no evidence to support the linear correlation of ScvO2 with the hemoglobin levels, there is great variability of ScvO2 at different hemoglobin level, we suggest the use of venous central saturation as a physiological marker for transfusions, avoiding with this practice making decisions only with the hemoglobin levels. }
